At Wellness Partners Mental Health, we are dedicated to offering comprehensive and supportive services for individuals facing mental health disorders, substance use disorders, or co-occurring conditions. Our Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP) provide structured therapeutic support tailored to the needs of adults who require intensive care but do not need 24-hour supervision.

Population Served


We proudly serve a diverse population, including individuals from various cultural, ethnic, and socioeconomic backgrounds. Our programs are inclusive and welcoming to all individuals, including members of the LGBTQ+ community.

 

Setting

Our PHP services are provided in person within a serene and therapeutic environment in Hawaii Kai, ensuring a peaceful setting conducive to healing and recovery. For convenience and accessibility, our IOP services are delivered online via secure virtual platforms, allowing clients to engage in therapy from the comfort of their own homes.

 

Hours and Days of Service

The PHP program operates Monday through Friday from 8:00 AM to 4:30 PM, offering both morning and afternoon sessions to accommodate various schedules. The IOP program also runs Tuesday through Thursday, with multiple sessions per week ranging from 3 to 5 days, each lasting 3 hours.

 

Frequency of Services

- PHP: Daily sessions, typically lasting 3.5 hours each.

- IOP: Multiple sessions per week, tailored to individual needs.

 

Payers and Funding Sources

We currently accept commercial insurance plans and private pay options. Fees include co-pays and coinsurance, making our services accessible to those in need.

 

Referral Sources

We accept referrals from a wide range of sources, including primary care physicians, psychiatrists, psychologists, social workers, hospitals, schools, employers, employee assistance programs (EAP), and self-referrals. While a referral from a healthcare provider is recommended, it is not required.

 

Specific Services Offered
  1. Therapeutic Interventions: Our program includes individual and group therapy sessions utilizing evidence-based approaches such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT).

  2. Psychiatric Services: Clients receive regular psychiatric evaluations and follow-up consultations from licensed psychiatric providers.

  3. Case Management: We provide comprehensive care coordination, including the development of individualized treatment plans and connecting clients with additional resources and services.

  4. Support Services: Access to support groups, educational workshops, and skill-building activities focused on enhancing daily living skills, stress management, and coping mechanisms.

  5. Family Involvement: We strive to include family members in the treatment process to support the client's recovery.

  6. Crisis Intervention: Immediate help for clients experiencing a mental health crisis, including safety planning and de-escalation techniques.

  7. Health and Wellness Education: Training on maintaining physical and mental health, including nutrition, exercise, and sleep hygiene.

    At this time, this program does not offer any mobile services.  

 

Service Provision Method
  1. Provided Directly: Core services such as therapy sessions, psychiatric evaluations, and case management are directly provided by our experienced staff, including licensed therapists, psychiatric mental health nurse practitioners, and case managers.

  2. By Contract: Specialized services or specific workshops, may be provided by external professionals contracted by the program.

  3. By Referral: Certain services, like housing assistance or vocational training, are facilitated through referrals to other organizations or providers.

Partial Hospitalization Program (PHP) Description

A Partial Hospitalization Program (PHP) is an intensive, structured mental health and substance use treatment program that offers a higher level of care than traditional outpatient therapy but is less intensive than inpatient hospitalization. PHP is designed for individuals who require significant support but do not need 24-hour supervision. Participants typically attend the program for several hours a day, multiple days a week.

The philosophy of PHP is rooted in the belief that individuals can achieve recovery and maintain their well-being while living at home and engaging in daily life. The program emphasizes a holistic, client-centered approach, integrating evidence-based practices to address the mental, emotional, and behavioral needs of clients. The goal is to provide comprehensive treatment that empowers individuals to develop coping skills, achieve stability, and enhance their quality of life.

  1. Stabilization: Achieve stabilization of acute symptoms of mental health or substance use disorders.

  2. Skill Development: Equip clients with coping strategies and life skills to manage their conditions effectively.

  3. Prevention of Hospitalization: Reduce the need for inpatient hospitalization by providing intensive outpatient support.

  4. Improvement of Daily Functioning: Enhance clients' ability to function in their daily lives, including work, school, and social relationships.

  5. Relapse Prevention: Develop and implement strategies to prevent relapse and maintain long-term recovery.

Individual Therapy: One-on-one sessions with a therapist to address specific mental health issues, set goals, and develop personalized treatment plans. Staff Credentials: Licensed Clinical Social Workers (LCSWs), Licensed Mental Health Counselors (LMHCs), Licensed Marriage and Family Therapists (LMFTs), Psychologists (PhD/PsyD), Pre-Licensed Therapists under supervision. 

​

Group Therapy: Facilitated group sessions that provide support, education, and skill-building opportunities. Common types include Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and psychoeducational groups. Staff Credentials: Licensed Therapists (LCSWs, LMHCs, LMFTs), Group Facilitators with relevant training and experience, Pre-Licensed Therapists under supervision.

Family Therapy: Sessions involving family members to improve communication, resolve conflicts, and support the client’s recovery. Staff Credentials: Marriage and Family Therapists (LMFTs), Licensed Therapists with family therapy training, Pre-Licensed Therapists under supervision.​

​

Psychoeducation: Educational sessions that provide information about mental health and substance use disorders, treatment options, and coping strategies. Staff Credentials: Licensed Therapists, Certified Educators, PMHNPs, Pre-Licensed Therapists under supervision, Registered Nurses.

Life Skills Training: Programs designed to teach practical skills such as stress management, communication, time management, and vocational skills. Staff Credentials: Licensed Therapists,  PMHNPs, Pre-Licensed Therapists under supervision, Registered Nurses.

Case Management: Assistance with accessing community resources, coordinating care, and developing discharge plans. Staff Credentials: Social Workers, Case Managers with relevant experience and training.

The staff in a PHP are highly trained professionals with appropriate credentials to provide specialized care. This multidisciplinary team typically includes:

  • Psychiatric Mental Health Nurse Practitioner (PMHNPs): Advanced practice registered nurses specializing in mental health, responsible for diagnosis, medication management, and overseeing treatment plans.

  • Psychologists (PhD/PsyD): Experts in psychological assessment, therapy, and research-based interventions.

  • Licensed Clinical Social Workers (LCSWs): Professionals trained in individual, family, and group therapy, with expertise in social support systems.

  • Licensed Mental Health Counselors (LMHCs): Clinicians skilled in providing therapeutic interventions for mental health and substance use disorders.

  • Licensed Marriage and Family Therapists (LMFTs): Specialists in family dynamics and therapy. 

  • Registered Nurse (RN): Healthcare professionals who provide medical care, monitor client health, and assist with medication management.

IOP
Intensive Outaptient Program (IOP) Description

​

An Intensive Outpatient Program (IOP) is a structured treatment program designed for individuals who require more support than traditional outpatient therapy but do not need the intensive care of an inpatient or partial hospitalization program. IOP typically involves multiple therapy sessions per week, focusing on mental health or substance use disorders, and allows clients to maintain their daily routines and responsibilities, such as work or school.

The philosophy of IOP is centered around providing comprehensive, flexible, and client-focused care. It aims to facilitate recovery and improve quality of life through evidence-based practices, while allowing clients to remain in their home environment. The program is designed to support individuals in achieving stability, developing coping skills, and fostering long-term recovery.
